Focused health-related case management, care coordination, and Husky-A presumptive eligibility application. Services are provided to eligible pregnant women and children up to three years old in the Southwestern area of Connecticut. The overall goals of this grant initiative are to reduce, to the extent possible, infant mortality and morbidity and low birth weight.
