Advocacy Services: Being the voice for individuals who need support in obtaining services.
Benefits Education Services: Programs designed to make residents aware of government or non-government programs available to assist them in meeting their needs and solving their problems. These programs address the details of the services provided, eligibility requirements and the places where services are delivered.
Case Management Services: Proper assessment of clients, which includes identifying barriers to employment, periodical reviews, and re-evaluations of employment plans. Assist clients in obtaining necessary documentations needed for State and Federal entitlements and provide supportive services.
Community Services: Cultivates community support, promotes the organization through community activities and assists with the implementation of special events.
Coordinate & Facilitate Services: Review social services components to ensure a balanced and enhanced delivery of services.
Supplemental Nutrition Assistance Training Program: Providing and finding every supportive service needed to facilitate an individuals’ transition into the labor market.
Healthy Start Program: Focused health-related case management, care coordination, and Husky (A) application assistance services to eligible pregnant women in the Southwestern area of CT.
Hispanic Elderly & Grandparents Programs: Services necessary to assure the health, safety and welfare of elderly people, as well as services that assist the elderly or aging persons to improve their quality of life. Services include social services, information and referral and case management.
Information & Assistance Services: Provides (A) current information on opportunities and services available to individuals in their communities, (B) assesses problems and capacities of the individuals (C) link individuals to opportunities and services that are available (D) ensures that individuals receive the services needed by establishing adequate follow-up procedures.
Nutrition Assessment Services: Service designed to increase public awareness and access via the State’s Supplemental Nutrition Assistance and Women Infant and Children programs.
Outreach Services: Initiate interventions for the purpose of identifying individuals with unmet assistance needs and encourage the use of existing services and benefits. This service includes both the initial contact and any follow-up contracts required to link the participant to appropriate services.
Social Support Services: Assist individuals in adjusting to problems in their personal lives or living environment.
