If you feel that you have been treated unjustly or improperly by one of our police officers, you may download and print the following forms (in English and in Spanish) and fill in as much information on it as you have available.

Once completed, the form should be submitted to the Bridgeport Police Department headquarters located at 300 Congress Street and directed to the Sergeant on duty. This form needs to be submitted in person to the Sergeant on Duty.
