The Bridgeport Health Department is responsible for managing all TB cases within its jurisdiction. This includes reviewing reports, interviewing patients, and approving discharge plans for those with TB. Please find below a copy of the discharge checklist, discharge treatment plan, and discharge procedure that can be used to ensure a smooth transition from the hospital.
