Bridgeport Health Department: Active TB Discharge Guidelines

The Bridgeport Health Department is responsible for managing all TB cases within its jurisdiction. This includes reviewing reports, interviewing patients, and approving discharge plans for those with TB. Please find below a copy of the discharge checklist, discharge treatment plan, and discharge procedure that can be used to ensure a smooth transition from the hospital. CADAP (Connecticut AIDS Drug Assistance Program)

CADAP (Connecticut AIDS Drug Assistance Program) helps eligible Connecticut residents living with HIV/AIDS access life-saving medications and related treatments. The program covers FDA-approved HIV medications and may also assist with health insurance premiums for qualifying individuals. CADAP is available at no cost to eligible residents and is designed to support ongoing care, improve health outcomes, and reduce barriers to treatment.

Healthcare providers may refer patients diagnosed with latent tuberculosis infection (LTBI) to our clinic using the referral forms below. Forms are available in English, Spanish, and Portuguese for your convenience.
