VAMS is a web-based tool being used across the United States to implement COVID-19 vaccination. The system provides:
- Organizations the ability to register eligible individuals for vaccination by roster
- Vaccine recipients the ability to schedule/modify appointments
- Access to healthcare facilities and clinics in the area providing the COVID-19 vaccine

FOR THE EMPLOYER/ORGANIZATION:
Complete the Connecticut Department of Health's application for VAMS Enrollment for Employers/Organizations (click here to access the DPH application)
After completing your application you will receive an email from VAMS@CDC.gov prompting you to create an account and sign-in verification for your organization
Create a roster of all individuals requesting to be vaccinated for COVID-19
Sort this list by eligibility using the current Connecticut vaccination phase (click here to view CT.gov's phases)
Upload current eligible employees to VAMS for enrollment

FOR THE VACCINE RECIPIENT:
After being enrolled by your employer/organization you will receive an email from VAMS@CDC.gov prompting you to create your account
When creating your account you will need to provide:
- Name, date of birth, race/ethnicity, home address, phone number and preferred method of contact
- Limited medical history (such as medications, if you are currently pregnant, and known allergies)
- Insurance information, if applicable
- After reviewing and confirming the information provided you will then be able to access the portal where you can schedule your appointment at one of several facilities providing vaccination.
- Please note at this time to access VAMS you can not use Internet Explorer. You can access VAMS using any alternative internet browser.

How do I change information I previously entered into VAMS?
You can log into VAMS at anytime to review, edit or update any of the account information you provided.
How do I reschedule/cancel my appointment?
Log into VAMS, under the "View Details" column select: "View Appointment". From there you can cancel/reschedule your appointment.
What happens if I miss my appointment?
The appointment is automatically cancelled after four hours if you missed it and VAMS will send a new message to schedule a new appointment.
How do I schedule my second dose
Once you complete your first appointment, VAMS will send a notification with a link to schedule a follow-up appointment.
